HIV Care Directions® (HIVCD), a program of the Area Agency on Aging, Region One, is a nationally accredited, professional case management program that has provided medical and support case management services to HIV positive people of all ages since 1992. The HIV Care Directions’ case managers conduct bio-psycho-social assessments, develop individualized care plans with each client, and provide linkages to services to meet each person’s identified needs. The case managers work extensively with medical providers, AIDS service organizations, government entities, and every community resource to meet those needs and accomplish an individual’s healthcare goals. Those services include access to and maintenance of primary medical care and medications, transportation for HIV care related services, and a variety of additional medical and support services for their clients.
